Template 3.1 (Appendix A) is helpful for identifying community partners to assist the Implementation Team with its efforts to promote organizational change. Using this template, the team can brainstorm possible partners and write down the expertise that each brings to the change initiative, how they might be involved, and ideas for engaging them. As part of its brainstorming efforts, the team can conduct Internet searches to obtain information about local organizations that advocate for or otherwise support SGM people and should talk to other people in the clinic and community for their suggestions.
