Chapter Summary

Action Plans are blueprints that break an organizational change initiative into discrete steps based on specific and measurable goals and objectives.

Action Plans outline what needs to be done, what resources are required, and who will do what by when. 

Action Plans account for the different implementation strategies to get a new policy or practice in place.

Plan-Do-Study-Act (PDSA) Cycles are useful for evaluating and leveraging the success of initiatives outlined in Action Plans.

Action Plans and the accomplishments they produce should be shared broadly and celebrated with providers, staff, patients, and others in the clinic’s community.
